Job summary

Interim HealthCare Inc. in Pratt, Kansas is seeking a Home Health and Hospice RN to deliver compassionate, patient-centered care in patients' homes. You will work with a multidisciplinary team to assess needs, coordinate services and advocate for each patient’s plan of care.

Qualifications

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases.
• Coordinate with physicians and oversee LPNs, CNAs and aides.
• Examine patients, perform assessments, document progress and report changes.
• Educate patients on plan of care, medications, therapy, diet and exercise.
• Coach family members on care plan, medications and safety at home.
• Ensure goals are met and coordinate discharge from services.

Duties

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
• Examine patients, do assessments, document progress and report changes to their physician
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
• Coach family members on the patient’s plan of care, medication and home safety
• Ensure goals are met and coordinate discharge from services

Requirements

• Graduate of an accredited nursing program and active RN license in Kansas
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills and compassion
