Job summary

RiseWell Community Services seeks a full-time Registered Nurse to join the Forensic Assertive Community Treatment (FACT) Team in Richmond Hill, Queens, NY. The RN will manage medications with the team psychiatrist and assess client health needs in a mobile, community-based setting.

The role focuses on delivering individualized care for persons with severe and persistent mental illness, prioritizing accessibility and comprehensive treatment in the community.

Qualifications

• Current licensure as a registered nurse by NYSED.
• Minimum 1 year of experience in a treatment or rehabilitation setting.
• A valid and clean NYS driver license.
• Fingerprinting and criminal background check.
• Approval from NYS Office of Mental Health.
• Strong documentation and organizational skills.
• Knowledge of nursing assessment methods.
• Ability to work as a generalist.

Responsibilities

• Conduct physical health assessments on clients.
• Assess physical health needs of clients.
• Make appropriate referrals to community physicians.
• Provide management and administration of medications with the team psychiatrist.
• Participate on a multidisciplinary team delivering integrated treatment and support services.
• Provide the majority of services in the community or at client-preferred locations.
• Utilize agency IT applications related to this position efficiently.
• Remain current on developments in the nursing field.

RiseWell Community Services is looking for a full-time Registered Nurses to join our Forensic Assertive Community Treatment (FACT) Team in Richmond Hill, Queens NY.

This individual will provide management and administration of medications, in conjunction with the team Psychiatrist, and assess the health needs of the clients. The Registered Nurse will serve as a member of a mobile, community-based, multidisciplinary treatment team for persons with severed and persistent mental illness, delivering individualized care to those in need.

QUALIFICATIONS

• Current licensure as a registered professional nurse by the New York State Education Department
• Minimum 1 year of experience in a treatment or rehabilitation setting
• A valid and clean NY State driver license
• Fingerprinting, criminal record check
• Approval from NYS Office of Mental Health
• Strong documentation and organizational skills
• Knowledge of nursing assessment methods
• Skilled in counseling techniques through education, training, or work experience
• Ability to work as a generalist
