RiseWell Community Services is seeking a full-time Registered Nurse to join our Young Adult ACT Team in West Babylon, NY. You will provide medical services on a community-based mobile, multi-disciplinary treatment team for young adults (ages 18-28) diagnosed with severe persistent mental illness.

The role includes management and administration of medications in conjunction with the Psychiatrist or NPP, ensuring medical standards per program regulations are maintained.

Qualifications

• Licensed RN in New York State.
• Minimum 1 year experience working with individuals with mental health diagnoses.
• Valid and clean NYS driver license.
• Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.

Responsibilities

• Provide medical services on a mobile, community-based ACT team for young adults (18–28) with severe persistent mental illness.
