Job summary

St. Joseph Hospice is seeking a Full-Time Hospice Registered Nurse (RN) in Shreveport, LA. The role requires an active Louisiana RN license, at least two years of RN experience (hospice or home health preferred), CPR, and solid EMR knowledge.

Responsibilities include patient assessments, developing the Plan of Care, educating patients/families, and acting as a liaison with the care team. On-site position with mileage reimbursement and comprehensive benefits.

Qualifications

• Registered Nurse (RN) license required in State of Louisiana.
• Minimum of two (2) years full‑time experience as an RN, in oncology, acute care with direct patient care, long‑term care facility, hospice or home health care; two years in hospice as an LPN may substitute for RN experience.
• Current CPR certification required.
• Current automobile insurance and valid driver’s license.
• Knowledge of EMR is required.
• Previous Hospice experience preferred.
• Compassionate RN with great time management skills.

• Performs the initial and on-going assessment of the patient.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, including medication administration, dressing changes, oxygen administration, Hospice philosophy and care of the terminally ill patient.
• Act as primary liaison between physician, patient/family and hospice team.
• Maintain an accurate and updated medical record.
• Makes routine supervisory visits.
• Participates in agency on-call schedule.
• Abide by company policies and procedures, compliance guidelines, applicable laws, and regulations promptly reporting any suspected violation of compliance standards.
