Job summary

St. Joseph Hospice is seeking a full‑time Hospice Registered Nurse (RN) in Sibley, LA to deliver compassionate in‑home and facility patient care.

The RN will assess patients, implement the Hospice Plan of Care, educate families, and coordinate with physicians and the hospice team to ensure high‑quality end‑of‑life care. Candidates must hold a Louisiana RN license, maintain CPR certification, and have at least 2 years of RN experience in relevant settings.

Qualifications

• RN license required in Louisiana.
• Minimum 2 years full-time RN experience in oncology, acute care, long-term care, hospice, or home health care; hospice experience preferred.
• CPR certification current.

Responsibilities

• Perform initial and ongoing patient assessments.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, medications, dressing changes, oxygen, and hospice philosophy.
• Act as primary liaison between physician, patient/family, and hospice team.
• Maintain accurate and updated medical records.

Qualifications

• Registered Nurse (RN) license required in the State of Louisiana
• Minimum of two (2) years full‑time experience as an RN in oncology, acute care, long‑term care, hospice, or home health care; two (2) years of hospice experience as an LPN may substitute for the required 2 years as an RN
• Current CPR certification required
• Current automobile insurance and valid driver’s license
• Knowledge of EMR is required
• Previous hospice experience preferred
• Compassionate RN with excellent time‑management skills

Job Duties

• Perform the initial and ongoing assessment of the patient
• Assist in developing and implementing the Hospice Plan of Care
• Educate patient and family on the Plan of Care, including medication administration, dressing changes, oxygen administration, hospice philosophy, and care of the terminally ill patient
• Act as primary liaison between physician, patient/family, and hospice team
• Maintain an accurate and updated medical record
• Make routine supervisory visits
• Participate in agency on‑call schedule
• Abide by company policies and procedures, compliance guidelines, applicable laws and regulations, promptly reporting any suspected violation of compliance standards
