Job summary

Preceptor Home Health & Hospice in Wisconsin is seeking a Registered Nurse (RN) Case Manager to lead patient care in Kenosha and Racine Counties. You will coordinate care with an interdisciplinary team, develop and update plans of care, and perform comprehensive assessments.

You will educate families, coordinate with physicians, supervise LPNs and Home Health Aides, manage documentation, and participate in discharge planning.

Qualifications

• Current Registered Nurse (RN) license in Wisconsin.
• Graduate of an accredited nursing program.
• Home health or public health experience preferred (not required).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and organizational abilities.
• Ability to work independently and manage a caseload.
• Valid driver’s license with reliable, insured vehicle.
• Physical ability to perform patient care tasks.

Responsibilities

• Develop, implement, and update individualized plans of care.
• Perform comprehensive patient assessments and ongoing evaluations.
• Provide skilled nursing care and execute physician orders.
• Educate and counsel patients, families, and caregivers.
• Coordinate care with physicians and other healthcare professionals.
• Manage documentation, primarily at patients' homes.
• Lead and participate in case conferences and care planning.
• Identify discharge planning needs and arrange services.
• Communicate patient status updates to the care team.
• Supervise LPNs and Home Health Aides; ensure equipment is arranged.
