Job summary

The Pennant Group, Inc. is seeking a Clinical Services Manager (RN) in Wisconsin to lead the clinical team across home health and hospice programs. This full-time role focuses on care coordination, staff guidance, and ensuring high-quality, patient-centered service delivery.

You will supervise referrals, plans of care, and interdisciplinary team meetings, while supporting staff development and maintaining compliance with F2F visits and quality initiatives.

Qualifications

• Current RN license in Wisconsin.
• BSN preferred.
• Minimum 2 years of leadership or supervisory experience.
• Home health or hospice experience preferred.
• Strong clinical judgment, communication, and organizational skills.

Responsibilities

• Managing patient referrals, reviewing cases, and assigning appropriate clinical staff.
• Overseeing the development, implementation, and updates of individualized plans of care.
• Collaborating with physicians and the Medical Director regarding patient eligibility and care needs.
• Supporting clinicians with goal setting, care planning, and clinical decision-making.
• Triaging and addressing concerns from patients, families, facilities, and providers.
• Approving and managing clinical workflows for home health and hospice services.
• Conducting home visits, record reviews, and performance evaluations to ensure quality care.
• Leading and participating in interdisciplinary team (IDG) meetings and case conferences.
• Performing chart audits, supervisory visits, and quality improvement initiatives (QAPI).
• Supporting hiring, onboarding, and development of clinical staff.
• Reviewing DME and pharmacy orders for accuracy and appropriateness.
• Monitoring outcomes and educating staff to support performance improvement.
• Managing compliance processes such as Face-to-Face (F2F) visits.
• Participating in leadership on-call rotation and completing visits as needed.
