Job summary

The Pennant Group, Inc.'s Wisconsin Nurses unit in Milwaukee is seeking an experienced RN to join a hospice/home health team. You will manage a 15-18 patient caseload and complete 5-6 visits per day on a standard Monday–Friday 40-hour schedule.

You will work within a collaborative, supportive IDG; there is no routine on-call on weekends, and there is a dedicated after-hours on-call team to assist coverage. A company vehicle is provided for visits, and Hospice experience is not required.

Responsibilities

• Manage a 15-18 patient caseload.
• Perform 5-6 patient visits per day.
• Follow Monday–Friday 40-hour schedule.
• Coordinate care with the IDG (interdisciplinary group).
• Participate in after-hours on-call coverage with a dedicated team.
• Operate with a company vehicle for visits.
• Serve Milwaukee County.
• Hospice experience not required.
