Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certification required.
• Must become knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Assume primary responsibility for patient and family caseload using the nursing process (assessment, admission, planning, implementation, evaluation).
• Collect data from patient and families across physical, psychological, social, and spiritual factors and coordinate care.
• Communicate with attending physician and interdisciplinary team to coordinate optimal care.
• Provide regular visits to reassess needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, progress notes, and care plans.
• Supervise and document home health aide delivery of care as required.

What Youll Do

• Assume primary responsibility for patient and family caseload including the assessment determining eligibility admission processing planning implementation and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical psychological social and spiritual factors that may impact the patient and familys needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re assess the patients needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records charts progress notes reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal State and Company guidelines

Qualifications

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
