Job summary

Qualifications

• Pass drug screening.
• Valid Driver's License required.
• Own reliable transportation to travel throughout Prosper and surrounding areas.
• Proof of car insurance.
• Social Security Card or Birth Certificate.
• Negative TB skin test.
• Provide three work-related references.

Responsibilities

• Conduct onboarding in-home assessments for clients seeking in-home support services.
• Create non-medical care plans including activities of daily living, light housekeeping, personal care, and other services.
• Onboard new clients and periodically reassess progress with in-home care services.
• Educate patients and families on non-medical in-home care services and scheduling.
• Utilize AxisCare EMR to accurately document patient information.
• Schedule initial caregiver visits.

• Conduct Onboarding in home assessments of clients seeking support services at home


• Create non-medical care plans to include activities of daily living, light housekeeping, personal care, medicationreminders, community transportation, respite care, etc.


• Onboard new clients and periodically reassess client's progress with thein-homecare services


• Educate patients and their families on non-medicalin-homecare services and scheduling


• Utilizeelectronic medical record systems (AxisCare) to accurately document patient information


• Scheduleinitialcaregiver visits
