Hospice of Virginia is seeking an RN Case Manager to support Forest and surrounding counties. You will join a growing team of clinicians delivering compassionate in-home end-of-life care.

Responsibilities include patient and family assessment, care planning, coordinating with the interdisciplinary team, and regular visits. We offer comprehensive benefits including PTO, 401(k) match, and competitive salaries.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process.
• Obtain data and assess information from patient factors that may impact needs and coordinate interventions with the interdisciplinary team.
• Initiate communication with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care.
• Provide regular visits to re-assess the patient's needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and care plans timely.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.

What You’ll Do

• Perform clinical pronouncement of death and provide support to the family
