Hospice of Virginia is hiring an RN Case Manager to support Forest and surrounding counties. Join a compassionate team of clinicians dedicated to end-of-life care with generous benefits and competitive salaries.

Monday–Friday 8a–5p with rotating on-call; mileage reimbursement and tuition assistance are offered. The role involves caseload management, coordination with interdisciplinary teams, and regular patient visits to assess needs and update care plans, with strong emphasis on quality patient

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid Certification.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsabilités

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process.
• Obtain data from patient’s physical, psychological, social, and spiritual factors that may impact needs and initiate intervention by the interdisciplinary team.
• Initiate communication with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care.
• Provide regular visits to re-assess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, and care plans timely.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.

What You’ll Do:
