Suncrest Hospice is seeking a Registered Nurse to provide in-home and facility-based patient care as part of an interdisciplinary team. The RN will perform psych-social assessments and connect patients and families with community resources, supporting Case Management, Admissions, or Resource roles.
You will travel across the service area, maintain documentation, and collaborate with nurses, social workers, and chaplains to ensure high-quality end-of-life care.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
