Job summary

Crossroads Hospice is seeking an RN Case Manager to provide compassionate in-home and facility care, coordinating with families and the interdisciplinary team to honor patient wishes and maintain dignity. You will build trusting relationships and serve as the central point of communication for our holistic care approach.

Responsibilities include delivering patient care in homes and facilities, collaborating with Clermont County/Batavia areas, and contributing to a schedule that supports patients

Qualifications

• 6 months minimum prior skilled nursing experience.
• An active unencumbered state Registered Nurse license.
• Current driver's license and auto liability coverage.
• Interest and ability in providing patient care in homes and facilities.

Registered Nurses, in our Case Management team, provide more than just medical assistance; they also listen attentively to patients' needs and honor their wishes. RN - Registered Nurse Qualifications

• No Mandatory On-Call Required
