Job summary

Adena Hospice seeks a Registered Nurse Case Manager (RNCM) to lead patient assessments and develop plans of care within hospice services. The RNCM will oversee clinical care, coordinate multiple disciplines, and ensure quality, cost-effective, billable care under physician supervision.

The role requires an active RN license in the employment state, with hospice experience preferred and a patient-focused, compassionate approach to care and family support.

Qualifications

• One year RN experience in an acute care or home care setting required.

Monday - Friday, 8:00am - 4:30pm; On-Call Every 20th Weekend; 1 Holiday Every 2 Years.

The Registered Nurse Case Manager (RNCM) - Hospice assumes ultimate accountability and leadership for the assessment of the patient condition and plan of care. The RNCM provides case management, clinical care and is responsible for coordination all disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician. ASHN adopts the RN scope of practice as outlined by the state for its licensed RN personnel.

QUALIFICATIONS

• One year of experience as a RN in an acute care or home care setting is required.
• Hospice experience is preferred.

EDUCATION AND CREDENTIALS
