Job summary

Crossroads Hospice & Palliative Care is seeking an experienced Registered Nurse Case Manager to provide compassionate in-home and facility-based patient care, coordinating with families and the interdisciplinary team to honor each patient’s goals.

You will build trusting relationships, ensure patients’ wishes are respected, and contribute to comprehensive care plans. Monday–Friday, 8:30 AM–5:00 PM, with no mandatory on-call; benefits include health, dental, vision, 401k, PTO.

Qualifications

• Minimum 6 months of prior skilled nursing experience.
• Active unencumbered state RN license.
• Current driver's license and auto liability coverage.
• Interest and ability in providing patient care in homes and facilities.

Responsibilities

• Build trusting relationships with patients and families as the central point of communication within the care team.
• Provide compassionate patient care in home and facility settings while coordinating with interdisciplinary team.

Registered Nurse Case Manager Qualifications:

• 6 months minimum prior skilled nursing experience.
• An active unencumbered state Registered Nurselicense.

Registered Nurse Case Manager Schedule & Benefits:
