Job summary

St. Joseph Hospice in Covington, LA is hiring a Full-Time Registered Nurse (RN) to join its hospice team. The role focuses on patient assessment, Plan of Care implementation, and family education, with strong collaboration across the care continuum.

The successful candidate will hold an active Louisiana RN license, have CPR certification, and bring 2+ years of full-time RN experience, preferably in hospice or home health settings. ESOP participation reflects the company ownership culture.

Qualifications

• RN license held in Louisiana.
• Minimum two years full-time RN experience in oncology, acute care with direct patient care, long-term care, hospice, or home health.
• Current CPR certification. Driver's license and auto insurance required.
• EMR knowledge required; hospice/home health experience preferred.

Responsibilities

• Performs initial and ongoing assessment of the patient.
• Assists in developing and implementing Hospice Plan of Care.
• Educates patient and family on Plan of Care, including medications and care tasks.
• Acts as liaison between physician, patient/family and hospice team.
• Maintains accurate and updated medical records.
• Conducts supervisory visits as needed.
• Participates in agency call schedule and adheres to compliance.

Qualifications

• Registered Nurse (RN) license required in the State of Louisiana
• Minimum of two (2) years full-time experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care. Two (2) years hospice care experience as an LPN may be substituted for the required 2 years of experience as an RN
• Current CPR certification required
• Current automobile insurance and valid driver’s license
• Knowledge of EMR is required
• Previous Home Health or Hospice experience preferred
• Compassionate RN with great time management skills

Job Duties

• Performs the initial and on-going assessment of the patient
• Assist in developing and implementing the Hospice Plan of Care
• Educate patient and family on Plan of Care, including medication administration, dressing changes, oxygen administration, Hospice philosophy and care of the terminally ill patient
• Act as primary liaison between physician, patient/family and hospice team
• Maintain an accurate and updated medical record
• Makes routine supervisory visits
• Participates in agency call schedule
• Abide by company policies and procedures, compliance guidelines, applicable laws, and regulations promptly reporting any suspected violation of compliance standards
