Job summary

Ebenezer is seeking an RN to provide personal care to adults in our Bloomington, MN skilled nursing facility. This role involves ensuring care plan alignment, coordinating medications, and documenting care activities in our charting systems.

Responsibilities include assessing resident goals, updating care plans when needed, and reporting changes to the team. Must hold MN RN license with CPR/BLS, and BLS grace period; evening shifts and every other weekend are required.

Qualifications

• Current MN RN license or by start date.
• AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers.
• 3-week grace period for BLS.
• Experience in geriatric care preferred.

Responsibilities

• Monitors personal cares to align with each resident's Care Plan.
• Coordinates medications and treatments per physician orders.
• RN assesses care plans and revises goals as needed.
• Provides reports and documentation of incidents and status changes.

Job Overview

Ebenezer is looking for a Registered Nurse - RN whom would provide personal cares to improve and/or maintain the adult residents’ physical and emotional well-being for our patients in our Bloomington, MN skilled nursing facility location, Martin Luther Care Center TCU and LTC!

Nursing Schedule

• 32 hours in two weeks
• Evening shift
• Every other weekend

Monitors personal cares provided to assure consistency with each individual Care Plan

Coordinates medications and performs treatments per physician orders and facility policies

RN – Assesses care plans, resident goals and revises, modify as needed

Provides reports and documentation related to incidents, change of status and cares provided

Required:

• Current MN RN license or by start date
• AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers
• (3 week grace period for BLS)

Preferred:

• Comfortable using Microsoft windows and electronic charting systems
• Work experience in a patient support role, LPN or RN
• Good written and verbal communication skills to get along with residents, families and physicians or providers
• Ability to work with and have sensitivity to the needs of the geriatric population.
