Job summary

Care Connection Rehabilitation Center in Rahway, New Jersey, is seeking skilled Registered Nurses (RNs) to join our patient-centered team. You will provide direct nursing care, follow individualized care plans, and collaborate with physicians and CNAs to ensure resident safety and comfort.

The ideal candidate holds an active New Jersey RN license, prior experience in long-term care, strong communication skills, and a compassionate, resident-first mindset.

Qualifications

Responsibilities

• Deliver direct, resident-centered nursing care.
• Coordinate with physicians, interdisciplinary team, and families.
• Monitor residents’ conditions and document changes.
• Administer medications and treatments accurately and safely.
• Maintain infection control and regulatory standards.
• Mentor CNAs and support team collaboration
