Job summary

Care Connection Rehabilitation Center in Rahway, New Jersey is hiring a Licensed Practical Nurse (LPN) to join our resident-centered, team-oriented facility. You will administer medications, follow individualized care plans, and perform wound care and other procedures to ensure residents receive high-quality care.

The role emphasizes accurate documentation, HIPAA compliance, and compassionate support to residents.

Qualifications

• Candidate must demonstrate strong patient care abilities and empathy.
• Familiarity with HIPAA and privacy practices is required.
• Previous skilled nursing facility experience is preferred but not required.

Responsibilities

• Administer medications, treatments, and supplies with accuracy.
• Develop and follow individualized care plans for residents.
• Provide wound care, treatments, and procedures as needed.
• Maintain clear, detailed medical records of treatments and observations.
• Protect patient confidentiality in accordance with HIPAA requirements.
