Job summary

Aspire Home Healthcare is seeking a Registered Nurse (RN) for home health in California. You will provide direct patient care and case management, conducting admissions, creating care plans, and coordinating with the interdisciplinary team to ensure high-quality home-based nursing services.

Responsibilities include patient assessments, education on care plans, and maintaining HIPAA compliance while delivering skilled nursing care.

Qualifications

• Active CA RN license required.
• Experience with OASIS and WellSky (Kinnser) preferred.
• Med-surg and wound care experience is a plus.
• Current physical exam within 6 months.
• TB test within 12 months and CPR certification.
• Valid driver's license required.

Responsibilities

• Provide direct patient care and case management.
• Assess patient condition, vitals, and mobility.
• Educate patients/families on care plans and home health services.
• Deliver skilled nursing care, HIPAA compliant.
• Document and submit visit records promptly.
• Develop and update individualized care plans.
• Coordinate with LPNs and Home Health Aides.
• Coordinate referrals for therapy or medical social work.
