Qualifications

• Active RN license in California.
• Experience with OASIS and WellSky (Kinnser) strongly preferred.
• Med-surg and wound care experience a plus.
• Current TB test (within last 12 months) and CPR certification.
• Valid driver’s license.

Responsibilities

• Provide direct patient care and case management per physician orders.
• Perform admissions, develop care plans, and coordinate with the interdisciplinary team.
• Educate patients and families on home health services and care plans.
• Deliver skilled nursing care while maintaining HIPAA compliance.
• Document visit records promptly and accurately.
• Coordinate with LPNs and Home Health Aides; facilitate referrals for additional services.
• Evaluate the effectiveness of nursing services and adjust as appropriate.
