Job summary

Aspire Home Healthcare in San Diego is seeking a Registered Nurse (RN) to provide direct patient care and case management in home health settings. You will perform admissions, develop care plans, and coordinate with interdisciplinary teams to ensure high-quality care.

Qualified candidates have an active CA RN license, experience with OASIS and WellSky, and a strong background in med-surg and wound care, with CPR and TB requirements up to date. Orientation will be held at the San Diego office.

Qualifications

• Active RN license in CA.
• Experience with OASIS and WellSky (Kinnser) strongly preferred.
• Med-surg and wound care experience a plus.
• Current physical exam within last 6 months.
• Current TB test within last 12 months and CPR certification.
• Valid driver’s license and reliable transportation.
• Must be able to attend orientation at the San Diego office.

Responsibilities

• Provide direct patient care and case management per physician orders.
• Conduct admissions, develop care plans, and coordinate with the care team.
• Obtain medical history, assess patient condition, and document findings.
• Educate patients and families on home health services and plans.
• Deliver skilled nursing care while maintaining HIPAA compliance.
• Coordinate referrals for therapy or medical social work and update visit records.
