The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team. You will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life.

Monday–Friday, 8:00 AM – 4:30 PM with on-call opportunities and additional compensation. Join a supportive team that values clinical excellence and compassionate end-of-life care in diverse settings.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsabilités

• Provide skilled nursing care, assessments, and symptom management per hospice standards and physician orders.
• Complete scheduled visits, admissions, and clinical interventions with timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and the inter-disciplinary team to coordinate care and support positive patient outcomes.
• Educate patients and families on symptom management and end-of-life care.

Schedule & Work-Life Balance
