The Care Team Hospice in Youngstown, OH is seeking a compassionate Hospice RN Case Manager to coordinate patient care, develop individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort and quality of life.

You will provide skilled nursing visits, document in Homecare Homebase, and support families through end-of-life care. Schedule is Monday–Friday, 8:00 AM–4:30 PM, with on-call opportunities that provide additional compensation.

Qualifikationen

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Schedule & Work-Life Balance

• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end‑of‑life care while providing compassionate support.
