The Christ Hospital Health Network in Cincinnati, OH is seeking a registered nurse (RN) to provide patient-centered care within the hospital’s model of care, emphasizing Family-Centered care and team collaboration.

You will assess, plan, implement and evaluate patient care, coordinate with the care team, and participate in safety, documentation, and discharge planning to support patient and family needs.

Qualifications

• Active Ohio RN license is required.
• BLS certification is required prior to patient contact; ACLS within 6 months in step-down/ICU areas.
• All nurses hired after Jan 2010 must obtain a BSN within 5 years of hire.

Responsabilités

• Assess, plan, implement, and evaluate patient care per the model of care.
• Communicate SBAR and handoff information to team members.
• Initiate discharge planning and coordinate care with patient and family.
• Maintain safety, accuracy in documentation, and ongoing education.
