Qualifications

Responsibilities

• Develop and revise multidisciplinary care plans with patient and family.
• Communicate plans using SBAR and handoff processes.
• Provide ongoing patient and family education tailored to needs.
• Respond to emergencies with immediate, controlled actions.
• Maintain safety and infection control per hospital policy.
