Job summary

Develops nursing plans of care with patients and designated care partners in alignment with cultural values. Responsibilities include collaborating with inter-professional teams, participating in education, ensuring safety and quality, and contributing to discharge planning and care

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Knowledge of patient- and family-centered practice and ability to convey respect and collaborate with patients and families.
• Cultural awareness and ability to work with diverse backgrounds in care planning.
• Understanding of effective communication concepts and ability to engage patients and families in health decisions.

Responsibilities

• Interacts with co-workers, patients and families to deliver optimal healthcare.
• Identifies learning needs with patients and families and provides education.
• Uses information systems to communicate, document, and monitor care outcomes.
• Works within nursing and inter-professional teams with open communication and mutual respect.
• Participates in safety and quality improvement committees and implements changes.
• Supports precepting and ongoing professional development.
