Ambercare Hospice in Santa Fe, NM is seeking a compassionate RN On-Call Nurse to join our team. In this role, you will provide expert clinical care to patients and families during meaningful moments, ensuring comfort, dignity, and quality of life.

You will manage after-hours nursing triage and patient care, while working within a supportive interdisciplinary team. Competitive compensation with mileage reimbursement and merit increases are offered.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications are required.
• Knowledge of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance are required.

Responsabilités

• Own primary responsibility for patient and family caseload including assessment, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data from patient physical, psychological, social, and spiritual factors to inform care.
• Coordinate care by communicating with attending physician, interdisciplinary team, and other agencies.
• Provide regular visits to re-assess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records, charts, progress notes, and timely care plans.
• Supervise and document delivery of care by Home Health Aide per guidelines.

What You’ll Do

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Qualifications

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR, First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid driver’s license and proof of insurance is required
