Qualifications

Responsibilities

• Assume primary responsibility for patient and family caseload.
• Obtain data and assess information impacting patient needs.
• Initiate communication with the attending physician and team.
• Perform regular visits to reassess patient's needs.
• Maintain accurate patient records and charts.
