Ambercare Hospice is seeking a compassionate RN On-Call Nurse to join our Santa Fe, NM team. You will provide expert clinical care and support to patients and families, ensuring comfort and dignity during meaningful moments.

The role features a Friday 5:00 PM to Monday 5:00 PM rotation, with enhanced compensation opportunities for hospice-certified clinicians and mileage reimbursement. Comprehensive benefits support your wellbeing and professional growth.

Qualificações

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsabilidades

• Assume primary responsibility for patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process.
• Obtain data and assess information from patient physical, psychological, social, and spiritual factors that may impact needs and coordinate interventions with the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, and timely care plans.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.

What You’ll Do

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process

• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Qualifications

• Licensed as a registered nurse in the state of practice
