Job summary

Hartford HealthCare Medical Group is seeking an experienced Registered Nurse to join our team in Connecticut. You will provide office-based nursing care in collaboration with providers across multiple sites, assess patient needs, plan and implement care, and exercise independent nursing judgment as appropriate.

The role requires a CT RN license and CPR certification, with preferred 3+ years in a medical practice and experience with EMR systems.

Qualifications

• Current state of Connecticut RN license and CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software (EMR) is desirable.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and plans of care; ensures continuity of patient care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing interventions.
• Assists patients and families with concerns, respecting confidentiality and privacy.
• Travel to regional offices as needed.
