Job summary

Vineyards-At-Fowler is seeking a Registered Nurse to join our healthcare team delivering patient care across diverse medical settings. You will assess patients, administer treatments, and coordinate care plans in person across hospitals, clinics, and long-term care facilities.

Ideal candidates hold a California RN license with BLS and experience in hospital medicine, outpatient clinics, or nursing homes, plus EMR proficiency (Epic, Cerner, Athenahealth) and strong patient assessment skills.

Qualifications

• Valid RN license in the state of employment with current BLS certification.
• Proven experience working in hospital medicine, outpatient clinics, assisted living facilities or nursing homes.
• Demonstrated proficiency with EMR/EHR systems including Epic, Cerner or Athenahealth for accurate medical documentation.
• Critical care experience preferred—ICU or trauma center—along with familiarity in emergency medicine and urgent care settings.
• Skills in IV insertion/infusion therapy, phlebotomy, specimen collection/processing, and sterile processing techniques.
• Knowledge of medical terminology, anatomy/physiology knowledge, ICD-10 coding standards, CPT coding for procedures and treatments.
• Ability to perform physical examinations effectively while demonstrating excellent patient assessment skills.
• Experience working with individuals with disabilities or developmental delays is highly valued; hospice & palliative medicine or occupational health is a plus.

Responsibilities

• Conduct comprehensive patient assessments including vital signs, physical examinations, and health history reviews using EMR systems like Epic or Cerner.
• Administer medications via injections, IV infusions, catheterizations, and other methods following aseptic techniques and HIPAA regulations to ensure patient safety.
• Assist with diagnostic evaluations including laboratory specimen collection/processing, radiology procedures like sonography or mammography, and wound suturing when necessary.
• Manage complex cases involving acute pain management, infectious disease care, behavioral health needs, or geriatrics including memory care and Alzheimer’s/dementia care.
• Educate patients and families on health coaching, discharge planning, medication administration, nutrition, and chronic disease management tailored to individual needs.
