This role assures that the organization is in compliance with state licensing requirements and all applicable state and federal regulatory standards are continually met.
Responsibilities:
- Perform and submit Start of Care Assessments, Quarterly Reviews and Quarterly Supervisory Evaluations
- Oversee the needs of clients receiving services
- Document observation of the participant's condition and needs
- Provide education to patients and families on useful home health care practices and techniques
- Coordinate care with Physicians, Physical Therapists, Occupational Therapists, Aids and other individuals in the patient's care plan
- Provide hands on skilled nursing services, (e.g., wound care, ventilator support, trach assistance, etc.)
- Recommend strategies or devices that may improve quality of life for the participants
- Take verbal orders
- Make referrals to other disciplines, as indicated in patient's healthcare blueprint
- Oversee patient medication profiles
- Host Medication Administration Teachings and/or other recurring trainings/classes, when applicable
- Complete service plans and identify needs
- Investigate incidents, grievances and accidents
- Communicate timely and effectively with agency on changes and/or recommended adjustments
Requirements:
- Bachelor's degree in nursing from an accredited program
- Must have reliable transportation
- 3+ years with hands-on nursing in a home care setting.
- Basic Health and Tuberculosis Screening
- Able to lift up to 50lbs
- Professional license is current and in good standing
- Holds Current CPR (American Heart Association) Certification
- Capability to think and work both independently and with direction
- Must communicate and carry out tasks efficiently in a fast-paced and sometimes stressful environment.
- Ability to produce a clean and clear background check under the agency's name
- Must complete the company's onboarding process in full before being assigned cases

- Compensation varies depending on assignment(s).
The general pay breakdown is as follows:
- Start of Care Assessments ($100.00 per assessment)
- Quarterly Reviews ($70.00 per review)
- Quarterly Supervisory Evaluations ($65.00 per evaluation)
- Rendering of Service Visits ($60.00 per hour)
- and Admin Assistance ($50.00 per hour)
