ComForCare Home Care in Baltimore, MD is hiring RN Case Managers to conduct in-home assessments and provide ongoing client care and case management. We have immediate openings with flexible hours, including weekdays, evenings, and weekend shifts.

We seek a Maryland-licensed RN with CPR/First Aid, preferably with Home Health/Hospice/Geriatric experience. You will oversee care plans, coordinate with families and healthcare professionals, and ensure high-quality patient outcomes.

Qualifications

• Current Maryland RN license or multi-state licensure.
• CPR/First Aid Certification.
• Experience in Home Health, Hospice, and/or Geriatric Nursing preferred.
• Strong assessment and clinical decision-making skills.
• Ability to work evening and weekend shifts, as needed.
• Reliable transportation for home visits.
• Familiarity with infection control and patient safety procedures.

Responsibilities

• Conduct in-home assessments and reassessments.
• Create and revise individualized care plans.
• Provide medication administration and nursing interventions.
• Monitor and document changes in client health and status.
• Communicate regularly with clients, caregivers, and care teams.
• Initiate referrals to healthcare or community resources.
• Maintain timely, accurate documentation in accordance with regulations.
• Participate and facilitate caregiver in-service programs.
