Vail Health in Vail, CO is seeking a Registered Nurse Navigator in Radiation Oncology to guide the patient care life-cycle within the department, acting as liaison among care components and between patients, families and providers.

You will perform admissions assessments, coordinate referrals, develop care plans, reinforce education, and sustain a patient-centered relationship from diagnosis through post-care, with a focus on access and quality of care.

Qualifications

• 2 years of experience working directly with patients and providers.
• Associate Nursing Degree required.
• Licensed as a Registered Nurse in the state of Colorado or from a valid compact state.
• Basic Life Support (BLS) required; Oncology Certification preferred if Oncology.

Responsibilities

• Consult with patients by phone or in person to determine needs and referrals.
• Build a patient-family centered relationship.
• Provide initial admission assessment, screenings, and assess for barriers to care.
• Report to team.
• Attend and/or review provider appointments; reinforce education and information provided by caregivers.

About the opportunity:

The Nurse Navigator role is intended to guide the entire life-cycle of patient care within the department. The Navigator is the liaison between all components of care; providing information seamlessly to the patient/ family, and between care providers in every care setting the patient uses.
