Job summary

Loyola Medicine Transport, part of Trinity Health, seeks a Pulmonary Hypertension Program Coordinator to guide patients through care in the outpatient clinic. You will work closely with physicians, nurses, social workers, administrators, and financial coordinators to deliver comprehensive, precise care.

This role requires nursing expertise, IL RN license, and CPR; BSN is preferred. You will educate patients, manage data, and participate in program improvements, with travel to partner facilities

Qualifications

• 3 years of professional nurse experience; at least 1 year of transplant, outpatient or dialysis experience desirable.
• ADN required; BSN, bachelor's degree in nursing preferred.
• Current IL RN license.
• CPR (Cardiopulmonary Resuscitation) certification.
• National Certification as Transplant Coordinator or related specialty preferred.

Responsibilities

• Coordinate care using established clinical protocols and document in progress notes and EHR.
• Collaborate with physicians, social workers, financial coordinators and nursing staff.
• Manage triage calls and support precertification/authorization with insurance carriers.
• Educate patients and families; develop educational resources for care understanding.
• Support quality metrics and participate in program improvement activities.

What You Will Do

As the Pulmonary Hypertension Program Coordinator, you will serve as a primary resource and advocate for patients, guiding them through their care journey with expertise, empathy, and precision. Working primarily in the outpatient clinic, you’ll collaborate daily with physicians, nurses, social workers, administrators, and financial coordinators to deliver comprehensive, age-appropriate care during both wellness and illness. This is a highly visible role within a specialty program known for innovation, excellence, and exceptional patient outcomes.

Key Responsibilities

60% CARE COORDINATION

Become the central hub of communication and organization for pulmonary hypertension patients as you:

• Coordinate care using established clinical protocols.
• Review diagnostic tests, labs, and clinical data to support decision-making.
• Ensure accurate documentation in progress notes, flow sheets, the Phoenix database, and the EHR.
• Collaborate with a multidisciplinary team including physicians, social workers, financial coordinators, and nursing staff.
• Manage triage calls and partner with physicians to assess and address patient care needs.
• Participate in program and committee meetings.
• Submit clinical information to insurance carriers for precertification/authorization.
• Provide cross-coverage to fellow coordinators as needed.

10% PATIENT & STAFF EDUCATION

Help Patients And Families Better Understand Their Condition By

• Creating and maintaining educational resources for patients, families, staff nurses, and program teams.

10% DATA MANAGEMENT

Support Program Excellence By

• Collecting, reporting, and submitting required data to the Pulmonary Hypertension Association in alignment with accreditation standards.

10% QUALITY & ADMINISTRATIVE SUPPORT

Elevate Program Performance By

• Preparing quality metrics, attending protocol meetings, and supporting administrative initiatives.
• Participating in Quality Assurance/Performance Improvement activities to strengthen outcomes and operations.

OUTREACH & EXPERT CONTRIBUTION

Expand The Program’s Reach And Reputation By

• Participating in community outreach efforts and traveling to partner facilities as needed.
• Serving as a pulmonary hypertension content expert at local, regional, and national levels.

What You Will Need

• 3 years of professional nurse experience; at least 1 year of previous transplant, outpatient or dialysis experience highly desirable.
• ADN required; BSN, bachelor's degree in nursing preferred
• Current IL RN license
• CPR (Cardiopulmonary Resuscitation)
• Preferred: Specialty certification in clinical or functional area of nursing granted by a national nursing organization
• Preferred: National Certification as Transplant Coordinator or other related specialty preferred.

Compensation

Salary Range: $39.48 - 65.14 per hour

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
