Qualifications

Responsibilities

• Assess physical, social, and psychological status and gather information from the interdisciplinary team and patient/family.
• Develop and implement individualized plans of care, including education, referrals, and follow-up.
• Direct the care team, delegate tasks, and coordinate resources to deliver care.
• Evaluate patient responses, revise plans as needed, and ensure timely documentation.
• Maintain professional standards, ethics, and regulatory compliance in practice.
