• Coordinates care with interdisciplinary team and coordinates education and referrals.
• Directs the care team, delegates tasks, and manages time and resources.
• Evaluates patient responses and revises care plans as needed.
• Adheres to legal, ethical, and regulatory standards and documents accurately.
