Job summary

Ascension St. John’s Medical Center in Tulsa, OK, seeks an experienced Case Manager RN to lead care coordination within the Case Management department.

This full-time role supports patients from admission to discharge, develops discharge plans to prevent readmissions, and mentors junior staff to improve workflows. Minimum requirements include an Oklahoma RN license and case management credentials; Master’s degree is preferred.

Qualifications

• Licensed RN credential from the Oklahoma Board of Nursing required.
• ACMA Case Manager credential preferred.
• CCMC Case Manager credential preferred.

Responsibilities

• Mentor Colleagues: Help junior case managers with complex cases and prioritizing tasks.
• Improve Processes: Review case management data to identify trends and enhance workflows.
• Prevent Readmissions: Develop detailed discharge plans to lower 30-day readmission rates.
• Ensure Continuity of Care: Support patients from admission through discharge to prevent care gaps.

What Minimum Requirements You’ll Need

• Licensed Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.
• Case Manager credentialed from the American Case Management Association (ACMA) preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education

• High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree

Required. Master's Degree Preferred.

What additional preferences we're seeking

• 5+ years of RN experience
• 3+ years of Case Management experience
• Hospital discharge planning experience preferred
