• Assess patient physical, psychosocial, spiritual, cultural and discharge planning needs.
• Formulate a plan of care with patient/family and health team input; set realistic goals.
• Implement the plan through direct patient care, coordination and supervision of staff.
• Evaluate patient progress and update the plan of care as needed.
• Document assessments, planning, implementation and outcomes in the patient record.
