Job summary

Responsibilities

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulate a plan of care, including discharge planning, based on assessment and medical plan of care.
• Implement the plan of care through direct patient care, coordination and supervision of the care team.
• Evaluate patient progress toward goals and adjust the plan of care as needed.
• Document assessments, plans, implementations and evaluations in the patient record with timely, legible notes.
