The role includes discharge planning and patient education, with a strong focus on evidence-based practice. Required are an Illinois RN license and BSN; ADN may be accepted with commitment to obtain BSN within three years.

Qualifications

• BSN required as part of nursing education.
• Current State of Illinois Registered Professional Nurse license is required.
• American Heart Association BLS certification is required.
• Strong interpersonal skills and ability to collaborate with others are essential.
• ADN may be accepted if there is commitment to obtain a BSN within three years of employment.

Responsibilities

• Assess and record patient status and identify care needs on admission or point of care entry.
• Organize health data to ensure accuracy and accessibility.
• Identify problems needing immediate attention and validate diagnoses with the patient/family and health care team.
• Document diagnoses and feed into the care plan and outcomes.
• Begin discharge planning at admission and coordinate with patient/family and care team.
• Provide patient and family education based on evidence-based practice.
• Communicate and collaborate with the care team to update care plans and outcomes.
