Job summary

Qualifications

• Current IL RN license
• BLS certification required

Responsibilities

• Assess and record patient status upon admission or point of care entry.
• Organize health data so it is accurate and accessible.
• Identify problems needing immediate attention.
• Validate diagnoses with patient/family and care team.
• Document diagnoses to support outcomes and care plan.
• Initiate discharge planning and coordinate with patient/family and team.
• Provide patient and family education based on evidence-based practice.
• Communicate progress at multidisciplinary meetings for feedback.
• Refer to appropriate care teams for continuity of care.
