Job summary

The RN Patient Navigator at Massachusetts Health & Hospital Association serves as a central contact in critical care, facilitating communication, care coordination, barrier removal, and emotional support for patients and families.

You will ensure families understand diagnoses and treatment plans, coordinate ICU transitions, address barriers to access, provide follow-up and education, and collaborate with interdisciplinary teams to optimize care.

Qualifications

• Licensed Registered Nurse with credential from the Oklahoma Board of Nursing.
• CPR certification (preferred).
• 5+ years of Critical Care or ICU nursing experience (preferred).

Qualifications & Skills
