Responsibilities
- Construct and implement the plan of care using nursing knowledge and skill, integrating direct and indirect patient care activities related to diagnosis and co-morbidities.
- Coordinate the interdisciplinary plan of care for assigned patients, developing goals, treatment plans, and discharge plans tailored to individual patient and family needs.
- Conduct hourly rounds with purpose to enhance nurse communication, improve safety, and anticipate patient needs (e.g., pain management, repositioning, toileting), while proactively evaluating the environment for safety and patient-friendliness.
- Support and inform patients and families about hospital-specific components of the Patient Centered Care Delivery model, including policies on medical records, visitation, and promoting a quiet, safe environment.
