Job description

Responsibilities

1. Construct and implement the plan of care using nursing knowledge and skill, integrating direct and indirect patient care activities related to diagnosis and co-morbidities.
2. Coordinate the interdisciplinary plan of care for assigned patients, developing goals, treatment plans, and discharge plans tailored to individual patient and family needs.
3. Conduct hourly rounds with purpose to enhance nurse communication, improve safety, and anticipate patient needs (e.g., pain management, repositioning, toileting), while proactively evaluating the environment for safety and patient-friendliness.
4. Support and inform patients and families about hospital-specific components of the Patient Centered Care Delivery model, including policies on medical records, visitation, and promoting a quiet, safe environment.
