Job summary

Saint Luke’s Health System in the United States is seeking a PRN Day RN - Case Manager to provide routine home care visits and complete admissions while delivering compassionate, high-quality care. Schedule: PRN Day, 8a-5p, with a flexible workload and a license valid in MO/KS or a compact.

The role involves admissions, home and inpatient visits as needed, and coordination with the health care team to deliver care in accordance with the Nurse Practice Act.

Qualifications

• Active RN license in MO/KS or a compact
• BLS certification from AHA or Red Cross
• Driver's license required
• Family Care Safety Registry (Missouri DHSS) required
• Associate Degree in Nursing (RN) required

Responsibilities

• Perform admissions and routine home visits and inpatient hospital visits as needed
• Coordinate care for a group of patients and consult with other health care team members
• Provide nursing care in compliance with the Nurse Practice Act
• Delegate patient care tasks and supervise other patient care staff
• Manage a caseload of patients

The Opportunity

Is responsible for coordinating the care for a group of patients and consulting with other members of the health care team when indicated. Provides nursing care in compliance with the Nurse Practice Act.
