Saint Luke’s Home Care & Hospice is seeking a Transitional Care Nurse to join our team. The role covers Kansas City metro area and surrounding counties, requiring RN licensure in MO and KS and 2 years of experience in Home Health or Hospice.

This full-time field position involves driving within Kansas and Missouri with flexible scheduling and occasional weekend telemonitoring. BSN is preferred; a clean driving record is required.

Qualifications

• 2+ years of experience in relevant care settings.
• Basic Life Support (American Heart Association/Red Cross).
• Registered Nurse license in MO & KS.

• Educate and engage patients and families; coaching and counseling.

Saint Luke's Home Care & Hospice is seeking a Transitional Care Nurse to join our team!! FT; M-F Flexible scheduling one weekend every 1-2 months for telemonitoring Clean driving record required Field position driving/covering territories in Kansas and Missouri BSN required RN in MO & KS required 2 years of experience required Coverage area is Kansas City Metro, may assist in rural areas within Leavenworth, Jackson, Clay, and Platte counties Experience in Home Health, Hospice, Case Management preferred

Job Responsibilities

The Transitional Care Nurse TCN, under the direction of the Director of Operations, serves as a key point of contact for care coordination and transition management for Saint Luke's Home Care and Hospice patients who move from the acute hospital and facility to the home settings and within levels of care when already home. The TCN will support patient self management, advocacy and oversee education and engagement, coaching and counseling of patient and family. Cross setting communication, teamwork and collaboration will be required for patient centered care planning and the nursing process will be utilized for ongoing assessment and outcome setting for each population managed.

Job Requirements

Applicable Experience: 2 years Basic Life Support - American Heart Association or Red Cross, Registered Nurse - Various Bachelor's Degree
