Job summary

UnitedHealth Group in Bartlett, TN is seeking a Hospice Registered Nurse to provide compassionate, patient-centered care by assessing needs, coordinating care plans, managing symptoms, and educating families.

You will collaborate with the clinical team, perform initial assessments on admissions, maintain progress records, and make referrals as needed. Eligible candidates hold an RN license and CPR, with 1+ years of nursing preferred.

Qualifications

• RN license valid in the state where you work (Tennessee).
• CPR certification required.
• 1+ year nursing experience preferred.

Responsibilities

• Perform initial nursing assessment on new admissions.
• Complete, coordinate, and revise the total plan of care.
• Regularly communicate patient progress to the clinical manager, physician, and care team.
• Develop, prepare, and maintain individualized patient care progress records.
• Record pain/symptom management changes/outcomes as appropriate.
• Educate families on patient condition, needs, and care.
• Make referrals to other hospice care disciplines as needed.
• Perform on-call responsibilities and on-call services to patients/families as assigned.

About This Role

Requirements
