Job summary

UnitedHealth Group is seeking a Per Diem Registered Nurse Case Manager in Union City, TN, to oversee hospice care plans. You will perform initial nursing evaluations, coordinate care, and maintain detailed notes while collaborating with families, physicians, and the IDG to ensure quality patient outcomes.

The role requires an unrestricted RN license in the state, at least 1 year of clinical experience, and CPR certification (or ability to obtain within 90 days).

Qualifications

Responsibilities

• Make initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify physical, psychosocial, and environmental needs; reassess every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family response.
• Collaborate daily with patient/family, attending physician, and IDG members.

About This Role

As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. This role is ideal for those who are dedicated to providing quality patient care and making meaningful connections with patients and their families.

What You'll Do

• Make the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
• Identify the patient's/family's physical, psychosocial, and environmental needs and re-assess as needed, no less than every fifteen (15) days
• Initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
• Document problems, appropriate goals, interventions, and patient/family response to hospice care
• Collaborate with the patient/family, attending physician and other members of the IDG in providing patient and family care daily

Requirements

• Current unrestricted RN licensure in the state of practice

Experience

• 1+ years of clinical experience

Licenses & Certifications

• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
• Current CPR Certification or ability to complete within 90 days of hire preferred
